We have a tendency to avoid apologies. We'd rather let slip a soft "my bad" at most, and just try not to fuck up again. When someone apologizes to us, we view it as a prologue to the offender hopefully getting their shit together.

Depending on the severity of the failure, Japanese companies typically scapegoat expendable middle management to publicly eat crow. If the president of a major corporation is trotted out to apologize, you know it's a major fustercluck of epic proportions.
